PRINTABLE AYSO LINEUP CARDS

A template for pre-printing lineup cards is available as described below.  This template allows coaches to fill out a lineup card with player names, numbers, and team information that can be printed on a perforated lineup card.  This allows coaches to fill out a single game card and then make copies that can be used throughout the season.  Download the instructions for using the printable lineup cards and the template below.

To use the printable AYSO Official Lineup Cards you will need the following: 

• AYSO Official Lineup Card stock – available from the AYSO Supply Center (Reorder #CS004P-7)

• A printer that can print legal size paper

• Adobe Reader loaded on your computer.  If you do not have Adobe Reader you can download it free by following the guidance on the left
